This application will generate a Membership Query Form for HWA events - where participants can sign-up to receive more information about HWA Membership by scanning the QR code on the form. It replaces the old 'Sign-Up Sheet' where participants had to hardwrite their name and email address.
 
The image to the right is an example form (it does not contain a valid QR code). Your form will have a QR-Code Specific to your event.
 
To create a form for your event, enter a description of the event below and select 'Create Form'. If you've previously created a form for this event, you can re-print it by selecting 'Lookup Previous Forms'.
 
 

Generate a New Form   Description of Event: (ie: "StokerCon 2024" or "Vancouver Chapter November Speaker" ...) Maximum Length: 60 characters

Re-Print an Existing Form   If you've previously create a Form and Custom-QR-Code for an event, it is better if you look up and re-print the form. This makes it easier for HQ to track participation at events. Thanks.
